New Doctors:
Please contact 1-800-328-9648 or 651-483-6611. We will
send you a New Doctor Packet that will contain the
information and materials that you will need to send us
your first case.
In-town case pick-up:
Is your dental office located in the Twin City Metro
area? If so you can call and schedule a case pick-up for
the same day or for the next day. Contact us at
651-483-6611.
Shipping a case FedEx:
To get our FedEx number to use for shipping us a case
please contact us at
1-800-328-9648 or 651-483-6611. We
will provide you with the information you will need to
send us your case using our FedEx number.
